Paediatric Therapy Services - Professional Referrals
Professional Referrals to Therapy in Praxis Limited
We recognise case managers and commissioners are often managing complex care packages and need to pull together a team of professionals and support workers to meet their client's needs. We are confident in our ability to meet those needs to make a difference to our client's lives.
Our extensive experience has given us a working knowledge of the role of the case manager, the medico legal process and the ever changing commissioning landscape.
We appreciate your need for regular information regarding the client's progress. We provide treatment plans after assessment and at 3 monthly intervals. We use amongst other outcome measures GAS* and SMART goals** to predict and measure an individual's response to therapy and back this up with photographs and video to illustrate change over time.
Our growing reputation for achieving results has led to an increasing number of referrals from a wide geographical consensus and investment has enabled us to provide referrers with the same, high quality, community paediatric therapy services they have relied on; but where possible with more cost effective travel rates than could previously be provided by our Harrogate facilities .
Our mission as a Practice is to be your Therapy Provider of Choice for children and young people.
GAS is an acronym for Goal Attainment Scaling … Goal attainment scaling is a technique for expressing, on a common scale, the extent to which goals have been achieved e.g. as part of a student’s plan, the class program, or the school plan. GAS achieves an individualised, criterion-referenced measure of change, providing a clear expression of plans and outcomes. It is potentially responsive to small changes that might otherwise go unnoticed through anecdotal protocols.
SMART is an acronym for Specific, Measurable, Achievable, Realistic and Timed and is an accepted goal orientated tool used in rehabilitation. A SMART goal represents a snap shot of the anticipated progress of the patient in one area within a time frame, and should be viewed as a part of the continuum of treatment and management of the patient and is not an end goal.
